GENERAL SUMMARY:  Assures that all medical needs are met in accordance with proper procedures set forth by the Director of Nursing.

ESSENTIAL DUTIES AND RESPONSIBILITES includes the following.  Other duties may be assigned.

– Proper administration of all medications.  These medications shall be administered in accordance with orders of physician and shall be in accordance with proper procedures developed by Pharmaceutical Advisory Committee.

– Assume responsibility for the ordering and maintaining of adequate supply of medication for each resident and assuring that such medications are correct and adequately stored.

– Assume responsibility that all treatments such as enemas, irrigations, skin care, etc., be accomplished with proper procedures.

– Responsibility of special diets and observations of eating habits, likes, dislikes, weight gain or loss, and recommend changes to physician when needed.  This shall be done in cooperation with dietary department.

– Assume the responsibility of assuring that each resident’s chart is kept current, dated, and signed completely legible and available at all times.

– Assume the responsibility to insure that all immunizations, records, lab work, physical exams, consultation notes, and all required forms are in place with current dates and signatures.

– Participate in IDT staffing’s offering input and opinion and establishing realistic short-term and long-term goals and appropriate approaches to accomplish these goals.

– Provide immediate first aid treatment to residents and employees in case of accident or injury.

– Be aware and conscientious in reporting any potential health hazards, which affects the safety of the residents or employees.

– Design and review care plans no less than every three (3) months for each resident in cooperation with the other disciplines involved in the care of the resident.

– Continuously endeavor to improve the health and welfare of each resident.  To suggest and initiate with medical approval any techniques which will strengthen potential skills and maintain existing skills and lastly, foster the human dignity and personal worth of each resident and enhance the coping capacity of the resident and family.



– Assure continuity of care from evening to night shift.

– Administers medications and treatment as prescribed with strict adherence to doctor’s orders and compliance with Federal and State regulations.

– Administers immediate first aid to both residents and employees in the event of accident/incident.  Notification to 3-11 Registered Nurse Super-visor.  If in her absence or upon her direction, notify physician if needed for medical orders.  Assume the responsibility of submitting accident reports immediately to nursing off-ice, document on incident sheets in building those incidents of a trivial nature and those which have no potential for serious injury.  Assume responsibility for documenting in nursing notes the circumstances of such incident/accidents and any symptoms of any injuries and treatment deemed necessary.

– Report any symptoms of illness or injury to 3-11 Registered Nurse Supervisor.  In her absence or if directed by supervisor, notify physician if treatment other than observation is warranted and assure guardian notification if necessary.

– Report any incidents of suspected abuse or neglect immediately to Registered Nurse Supervisor.  In her absence or upon her directions, LPN shall be responsible for initiating a preliminary investigation in the event a suspected abuse or neglect occurs as possible and also the notification of a least one member of the internal Review Committee or the Administrator on duty.

– Performs general nursing duties as required.

– Assist all other members of the nursing team.

– Assure thorough follow-up, observation, and documentation and those clients who have had an injury as a result of incident/accident during the, day as well as those clients who have symptoms of illness during the previous shifts.

– To submit a shift report to the nursing office, which indicates those clients who have had significant changes in condition, changes in physician’s orders, any accidents/ incidents, or any other information, which will assure continuity of care.  Assume the responsibility of the documentation on all monthly weights and blood pressures of all clients in cooperation with the day shift of nursing personnel.

– Assist cottage staff with the dietary concern of their clients and monitor the client’s acceptance of diets.



– Assure continuity of care from days to evening shift.

– Administer medications and treatments as prescribed with strict adherence to doctor’s orders.

– Administers first aid as necessary to both residents and employees.  Proper notification to physician and notification of guardian if deemed necessary.

– Assume responsibility of reporting any symptoms of illness or injury immediately to physician and carry out orders as prescribed.

– Responsible for reporting any incidents of abuse or neglect immediately to the abuse committee members.  In their absence make a preliminary investigation with proper notification later.

– Performs general nursing duties as required.

– Assist all other members of the nursing team.

– Submit report to nursing office.

Education: Licensed Practical Nurse, graduate of accredited school of nursing, current and valid Illinois license, experience with the intellectually disabled preferred.

– Highly proficient computer skills and comfortable with software programs such as Microsoft Office Suite (word, excel, access, power point) or similar software.  Advance knowledge and use of Outlook.

Abilities Requirements:

Mathematical Skills: Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions, and decimals.

Reasoning Ability: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or report form. Ability to deal with problems involving several concrete variables in non-standardized situations.  Able to use tangible and intangible information to come to a result.

Technology Skills: Utilizes computer to accomplish job tasks.  Computer literacy essential in word processing, spreadsheet applications, and databases.  Knowledge of Outlook or similar program.

While performing the duties of this job, the employee is regularly required to sit, stand, walk, bend, squat, crawl, and reach.  Employee will occasionally use a mechanical lift, climb stairs and ramps.  The employee will frequently and occasionally lift and/or move up to 75 pounds.  From time to time, employee may be required to work long hours.  The employee is frequently required to use both hands, push/pull, temp changes use sharp objects, and talk.  The noise level in the work environment is usually moderate.  On occasion, noise level may be very loud.  On occasion the work environment may be cold and hot.

Specific vision abilities required by this job include close vision, distance vision, and peripheral vision.  Specific hearing abilities required by this job include the ability to hear when communicating with employees and using a phone line.
